Community Health Needs Assessment Data Dashboard
Explore some of the key data that informed the Community Health Needs Assessment using this dashboard. Although the CHNA report was finalized in 2025, this dashboard contains the most updated data available, and will continue to be updated regularly. Some of the indicators shown in this dashboard are health outcomes, such as rates of hypertension, and some are social determinants of health (SDOH), such as income. Social Determinants of Health are the community-wide social, economic, and physical conditions we experience where we are born, work, live, play, and age. The indicators in the dashboard can be compared by population groups such as age, sex, and race, depending on data availability.
There are differences in some of the indicators between population groups, particularly by race. This is not due to individual choices, but rather due to systematic oppression including structural racism. As the CDC states, “[Social determinants of health], including the effects of centuries of racism, are key drivers of health inequities within communities of color. The impact is pervasive and deeply embedded in our society, creating inequities in access to a range of social and economic benefits—such as housing, education, wealth, and employment.
